You will get superior support and guidance from a personalized program geared to meet your specific needs. You will leave each session with obtainable goals to work on during the weeks and months that follow.

** All services can be divided into a monthly fee for up to 6 months and paid incrementally with a credit card service.

Level I Nutrition Program

  • Consultation of 90 minutes
  • Assessment of 3 day food journal
  • Evaluation of current supplementation
  • Assessment of current health status and medical history
  • Two weeks of daily access to email or phone for further questions and support
  • 30 min office visit or phone call to present nutrition implementation plan, discuss progress and set goals
  • Follow up phone consult or email to patient’s physician if desired
  • The introductory program can be rolled over into any other plan. For example, if after two weeks, the patient would like more support, they can continue for two more weeks and pay the difference of the Level II plan (which is a lower rate per time). There is no obligation to progress to any other plan.

Level II Nutrition and Wellness Program

Studies show that one-on-one individualized nutrition programs yield the most effective results over time. If you have less than 20 pounds to lose, or have a metabolic condition like diabetes or pre-diabetes then this program would work for you.

  • Initial Consultation of 90 minutes
  • Higher level of accountability than the Level I Program
  • Assessment of 3 day food journal collected and analyzed weekly
  • Initial food journal is evaluated and recommendations for change are given
  • Meal planning
  • Evaluation of current supplementation
  • Supplementation recommendations given
  • Assessment of current health status and medical history
  • Presentation of nutrition implementation plan
  • Two Weekly office or phone consultations of 30 minutes to discuss progress and set goals
  • Four weeks of daily access to email for further questions and support
  • End-of-Program follow up session of 60 minutes
  • Follow up phone consult or email to patient’s physician if desired
  • You can start at any time during the month

Level III Nutrition and Wellness Program

On this Level III, you benefit from a high level of accountability and experience greater success with obtaining your health and weight loss goals.

  • Initial Consultation of 90 minutes
  • Higher level of accountability than the Level II Program
  • Assessment of 3 day food journal collected and analyzed weekly
  • Initial food journal is evaluated and a printed analysis is given
  • Meal planning
  • Evaluation of current supplementation
  • Supplementation recommendations given
  • Assessment of current health status and medical history
  • Presentation of nutrition implementation plan
  • Six Weekly office or phone consultations of 30 minutes to discuss progress and set goals
  • Eight weeks of daily access to email for further questions and support
  • End-of-Program follow up session of 60 minutes
  • Follow up phone consult or email to patient’s physician if desired

Level IV Optimal Nutrition & Lifestyle Program

This is truly the Rolls Royce of services and is all about creating optimal wellness for you. This program includes a high level of nutrition education, stress management, accountability and behavioral modification. A physical activity plan is also included. It is ideal for weight loss over 20 pounds, optimal wellness and challenging medical conditions.

  • Initial Consultation of 90 minutes
  • Higher level of accountability than the Level III Program
  • Ten Weekly office or phone consultations of 30 minutes to discuss progress and set goals
  • Assessment of 3 day food journal collected and analyzed weekly
  • Initial food journal is evaluated and a printed analysis is given
  • Meal planning
  • Evaluation of current supplementation
  • Supplementation recommendations given
  • Assessment of current health status and medical history
  • Presentation of initial nutrition implementation plan
  • Three months of daily access to email or phone for further questions and support
  • A personalized physical activity plan
  • End-of-Program follow up session of 60 minutes
  • Follow up phone consult or email to patient’s physician if desired

You are worth the investment to optimize your health. Lindy’s greatest joy is for her patients to experience nutrition and health success and enjoy a higher quality of life.

All the programs are heavily discounted. Buying the sessions individually would cost much more than what the packages offer.

Flexible and Health Savings Accounts and Health Insurance

Does your employer offer a Flexible Spending Account (FSA) or Health Savings Account (HSA)? You may have an HSA account if you are self-employed.

You can include in medical expenses amounts you pay to lose weight if it is a treatment for a specific disease diagnosed by a physician (such as obesity, hypertension, IBS, heart disease, etc). This includes weight management programs, if prescribed by a doctor. Although obesity alone is not covered under current insurance plans, it is covered in full under FSA and HSA accounts.

Step 1 – See your nutritionist, Lindy Ford

Step 2 – Call your primary healthcare physician

Inform them that you have an FSA/HSA account and you would like to use your pre-tax dollars for your weight management or nutrition therapy care as it relates to your medical condition. Below are the checkpoints the IRS requires to be included on the prescription. You will need to hold on to your prescription as well as your receipts for reimbursement and/or validation.

Step 3 – Submit your claim for re-imbursement

For re-imbursement, you will need to submit your claim along with the doctor’s letter. A doctor’s letter (prescription) is valid one year from the date written. After that, you will need to have a new prescription written.

Please be informed that the prescription should have the following information:

  1. Diagnosis
  2. Treatment duration
  3. Equipment / medication / therapy / program needed for treatment.
  4. Date of issue the DRs RX / Letter of medical necessity.
  5. For whom the treatment is prescribed.
  6. Doctor’s name and signature.
